Be Safe Healthcare, LLC ("Be Safe Healthcare," "we," "us," or "our") respects your privacy. This Privacy Policy explains how we collect, use, disclose, and safeguard information when you visit besafehealthcare.com, contact us, or interact with our services, including SMS and voice communications.

Protected Health Information (PHI) that Be Safe Healthcare handles on behalf of physician practices is governed by the applicable Business Associate Agreement (BAA) and HIPAA, not solely by this Privacy Policy.

2. Information We Collect

We may collect the following categories of information:

  • Contact information you submit through forms on our website, such as name, practice name, email address, and phone number.
  • Communications you send us by email, phone, or SMS.
  • Website usage data such as IP address, browser type, pages visited, and referring URL, collected through standard logging and analytics.

3. How We Use Information

  • To respond to inquiries and provide requested information.
  • To communicate with prospective and current practice partners about services, onboarding, and account matters.
  • To send appointment confirmations, reminders, care coordination updates, and operational notices via phone call or SMS where consent has been provided.
  • To operate, maintain, and improve our website and services.
  • To comply with legal obligations.

4. SMS / Text Messaging Program

Be Safe Healthcare uses SMS to communicate with practice partners, staff, and (where authorized) patients enrolled in Chronic Care Management. The following terms apply to all SMS communications from Be Safe Healthcare:

Consent

By providing your mobile number on our website, through a written enrollment form, or verbally to a Be Safe Healthcare representative, you agree to receive SMS messages from Be Safe Healthcare related to the services you have requested or are enrolled in. Message and data rates may apply. Consent is not a condition of purchase.

Message Types and Frequency

Messages may include appointment reminders, care plan check-ins, secure links to patient resources, onboarding updates, and operational notices. Message frequency varies based on your enrollment and interaction with the program.

Opt-Out

You may opt out of SMS messages at any time by replying STOP to any message you receive from us. After replying STOP you will receive one confirmation message and no further SMS messages will be sent, except as needed to support your care where you have separately authorized such contact.

Carriers and Delivery

Carriers are not liable for delayed or undelivered messages. Supported carriers include all major U.S. carriers. Message and data rates may apply based on your mobile plan.

No Sharing of Mobile Information

No mobile information, including phone numbers, SMS opt-in, or text message consent, will be shared with third parties or affiliates for marketing or promotional purposes.

Text messaging originator opt-in data and consent will not be shared with any third parties, excluding aggregators and providers of the text message services. Aggregators and messaging providers are contractually prohibited from using this information for their own marketing purposes. All other categories of sharing described elsewhere in this Privacy Policy specifically exclude SMS opt-in data and mobile phone numbers collected for SMS purposes.

5. How We Share Information

We do not sell personal information. We share information only as follows:

  • Service providers that support our operations (for example, hosting, email, telecommunications, and SMS delivery), under contracts that restrict their use of the information.
  • Physician practices that have engaged Be Safe Healthcare, where the information relates to services provided on their behalf.
  • Legal and safety reasons, including to comply with applicable law, respond to lawful requests, or protect the rights, property, or safety of Be Safe Healthcare, our clients, or others.

6. Data Security

Be Safe Healthcare maintains administrative, technical, and physical safeguards designed to protect information against unauthorized access, disclosure, alteration, or destruction. PHI is handled within HIPAA-aligned systems and under Business Associate Agreements with the practices we serve.

7. Your Choices

  • You may opt out of SMS messages at any time by replying STOP.
  • You may request access to or correction of personal information you have provided by contacting us using the information below.
  • You may disable cookies through your browser settings; portions of the website may not function as intended.

8. Children's Privacy

Our website and services are directed to healthcare practices and adult patients. We do not knowingly collect personal information from children under 13.
